We apologize for not sending out the conference survey email because of technical difficulties. That said, we have extended the deadline to Sunday October 25, 2009 at 11:59 pm.

This is was our first year working together and co-sponsoring this event with officers from AMSA ARC and AMSA UCD. We know the conference was not perfect, but we would love to have your feedback on how we can improve for next year. Please be honest as you can be. Our feelings won't get hurt, but we can strive to make the event better in the future.

Please note that we are pre-med students just like you, and we planned this conference on our own time; we are not paid, and we have limited financial support. We strived to give you as much as we can, but due to our limited means and the current economic crisis which makes fundraising that more difficult, we had to cut back on this year's budget.

This survey is anonymous. None of the medical schools, workshop presenters, conference planners, and speakers will know how you responded. We will use your contact information only to distribute raffle prizes.

We ask that you respond by Sunday October 25, 2009 at 11:59 pm to qualify for the Raffle items. The number of prizes gives everyone an 8% chance of winning a prize.

The names will be drawn on October 28, 2009 and we will contact via e-mail all the winners by November 1, 2009. We will also contact the grand-prize winner by phone as well. When contacted, we will be asking for a mailing address so we can mail your prize to you by certified mail via the United States Postal Service.

Rules of the Raffle

• We will draw the winning names from the pool of those, who responded to the entire survey.
• The names will be drawn at random.
• The smaller prizes will be picked first, and the grand prize will be picked last.
• One person can win only one prize.
• All prize items are non-transferrable.
• The MCAT prep courses CANNOT be given, transferred, or donated -- only the winners will be able to use them. (Their names and contact information will be given to Kaplan represenatives who will present them with course certificates.)
• The gift items from the medical schools and other organization are randomly selected. We will make the best effort to get you items that best match you (i.e. sending the right gender size shirt/sweater).
